An Original Civil War " Disability for Discharge" issued for Private Warren Fuller, 20th Indiana Volunteers Near Portsmouth (Virginia), June 1, 1862, discharged for "St Vitus Dance", sgd by Captain James H Shannon a Gettysburg survivor who became Colonel of the 138 Indiana.

Also sgd by notable Brigade Surgeon Josiah Curtis. The Discharge is signed on the reverse by noted American Artist Charles Temple Dix, the son of General John Dix.
