Civil War Soldier Document for Pay Dated 1862

This document was given to Ira "Carnfield" (can't make out the last name) who served in 1861 and was let go in 1862. The reason for being let go is atrophy of the right shoulder and arm.

This appears to be a certificate to be carried so they can receiver their pay.
